Responsibilities

Provide high-quality nursing care to residents under the direction of the RN and DON
Administer medications and treatments accurately and timely
Monitor and record resident health status and communicate changes promptly
Foster positive relationships with residents, families, and interdisciplinary team members
Ensure compliance with all facility policies and state/federal regulations

Requirements

Valid Ohio LPN license in good standing
Previous long-term care or rehabilitation experience preferred
Strong communication and teamwork skills
Compassionate, reliable, and committed to quality care
