As a Licensed Practical Nurse, you will be administering skilled nursing care to patients on an intermittent basis in the patient's place of residence in close coordination with other disciplines and caregivers involved in the patient's care. This is performed in accordance with physician orders and plan of care and in accordance with agency policies and procedures.

Qualifications:

• Completion of a practical nursing program
• Current LPN licensure in the state of Pennsylvania required
• Previous Home Health/Hospice experience preferred
• Current PA drivers license and insurance
• CPR certification from the American Heart Association within the first year of hire required
