The Client Care Advisor (CCA) is a relationship‑driven role at the center of our care team. You’ll work closely with clients, families, Care Professionals, and internal teammates to make sure care feels thoughtful, steady, and well‑coordinated.
What This Role Is Really AboutThis client care coordination role supports older adults and their family through care consultations, care planning, coordination, and ongoing quality support in a home care setting.

You’ll balance people‑first connection with clear follow‑through — helping everyone stay aligned as needs evolve. Some days are predictable, others require flexibility, and you’ll be supported by a team that values collaboration over perfection.
What You’ll Spend Your Time DoingMeeting with clients and families to conduct care consultations
Creating and updating personalized care plans
Being a friendly, consistent point of contact for clients and families
Working closely with Scheduling and Recruitment to support strong care matches
Leading caregiver introductions and early follow‑ups
Checking in regularly to ensure care remains high quality
Supporting transitions like hospital discharges or changes in care needs
Documenting clearly and accurately in Well Sky and Salesforce
Enjoy interacting with people throughout the day
Are approachable, friendly, and easy to work with
Like being part of a team and contributing to shared goals
Are comfortable adjusting your pace as the day unfolds
Can follow processes while knowing when flexibility is needed
Want to make a real difference without constant chaos
Client care coordination, healthcare, social services, or customer‑focused roles
Strong communication and organizational skills
Comfort working with older adults and their families
Ability to manage multiple relationships at once
(If you’ve succeeded in people‑centered roles but don’t come from a traditional healthcare background, we’re open to that.)
