Main duties of the jobAs care coordinator you will play a central role in ensuring safe high quality person-centred care across our services you will work closely with operational and quality teams offering clinical leadership and supporting best practice in line with CQC standards

- Provide clinical leadership across supported living services.
- Review and implement care plans for individuals with complex or high‑risk needs.
- Act as the lead clinical liaison for referrals, transitions and MDT meetings.
- Support service managers to embed best practice and maintain CQC compliance.
- Monitor incident reports, safeguarding alerts and health outcomes.
- Lead on health audits, clinical risk assessments and medication reviews.
- Train and mentor support staff in trauma‑informed, high‑quality care.
- Work collaboratively with local authorities, NHS commissioners and families.
- Minimum 3 years post registration experience in a clinical or Care Coordination role.
- Experience in supported living residential care or community adult health.
- Strong understanding of CQC, DoLS, MCA, and risk management frameworks.
- Excellent communication, leadership, and organisational skills.
- Ability to travel between services (mileage reimbursed).
- Experience with forensic clients or dual diagnosis.
- Experience supporting services through CQC inspections.
- NMC Registered Nurse (RMN, RNLD, or RGN with community/complex needs experience).
