Kingswinford and Wordsley PCN are looking to recruit a new to General Practice Nurse within the ARRS roles to join our multi-disciplinary team, to help us to transform and modernise pathways of care, enabling the safe and effective sharing of skills across traditional professional boundaries.
Main duties of the jobRole may require Saturday availability to support with PCN events.
Practice Nurse with competency in long-term condition management.
Role involves managing long-term conditions and caring for elderly, frail, housebound, or care home patients.
Must have own transport and ability to drive.
Capable of managing:
- Cervical Cytology
- Childhood Immunisations
- Family Planning
- Respiratory care(spirometry training preferred)
- Removal of Sutures
- Injections and vaccinationsin line with the national immunisation programme
- General nursing duties as required
- Provide comprehensive nursing care within the GP surgery setting
- Deliver health education and counselling to patients and their families
- Maintain accurate clinical records using EMIS
- Conduct telephone and face-to-face consultations
Kingswinford & Wordsley Primary Care Network is a network of 5practices with a list size of over 50,000 patients. This role exists tosupport the delivery of the Kingswinford & Wordsley Primary Care Network agenda. The post will form an integral part of the PCN leadership team working alongside the Practice Managers and the Clinical Director and the PCN Partners.
Job responsibilitiesLong-Term Condition Management & Elderly Care
Manage long-term conditions such as hypertension, diabetes, asthma, COPD, and cholesterol management.
Provide care for elderly, frail, and housebound patients, including those in care homes.
Conduct home visits as required.
Support quality improvement and assurance initiatives within the PCN.
General Practice Nursing Duties
Provide immunisations, including:
Routine influenza, pneumococcal, shingles vaccinations.
Infant immunisation and travel health care vaccinations.
Perform cervical smears, pregnancy tests, and contraceptive advice.
Assist GPs with minor surgeries and coil fittings if required.
Carry out ECGs, venepuncture, urinalysis, and wound care (including ulcer management and suture removal).
Review and action pathology tests such as urine culture and swabs where appropriate.
Chaperone and assist patients being examined by other clinicians.
Team Support
Nursing associates, and HCAs.
Work collaboratively with the wider practice team to enhance patient care.
Promote public health and screening programs, including immunisations and cervical screening.
Integrate population health management approaches to reduce health inequalities.
Clinical Governance & Compliance
Maintain accurate and up-to-date patient records.
Ensure compliance with clinical protocols, including prescribing audits and reviews if qualified.
Adhere to NMC revalidation requirements and practice within the NMC Code of Conduct.
Follow infection control policies and ensure safe handling of specimens.
Contribute to audits and service evaluations.
Support the effective use of NHS resources and practice policies.
Ensure the cleanliness and tidiness of nursing and treatment rooms.
Maintain and clean clinical equipment.
Monitor and order medical stock, drugs, and vaccinations.
Attend and contribute to practice meetings and training sessions.
Health & Safety Responsibilities
Comply with national infection control standards and health and safety guidelines.
Ensure safe handling, storage, and transport of specimens.
Identify risks in work activities and take necessary precautions.
Person Specification Experience- Experience of working in a primary care environment;
- Experience of working autonomously;
- Experience of infection prevention and control measures;
- Clinical knowledge in the following:
- Wound care and removal of sutures and staples
- ECGs
- Venepuncture
- New patient medicals
- Immunisations (routine, childhood and travel)
- Requesting pathology tests and processing the results, advising patients accordingly;
- Travel medicine (or willing to learn);
- Chronic disease management awareness for:
- CHD
- Asthma
- COPD
- Diabetes
- Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications;
- Post graduate diploma or degree (Chronic Disease Management);
- Immunisation training (or willing to learn);
- Cytology training (or willing to learn);
- Family planning qualification;
- Experience of working as a practice nurse or community nurse;
- Experience of managing elderly and housebound patients;
- Experience of quality initiatives, i.e., benchmarking.

- 5 GCSEs, 4-9 grades, including:
English, science and maths. - 2-3 A levels or BTEC Level 3 Diploma.
- Hold an academic Level 6 diploma certification in long-term conditions care.
- Undertaking the Fundamentals of General Practice Nursing Programme.
- Registered with the NMC and maintains revalidation in line with NMC requirements.
