Heart Failure Specialist Nurse

Job Overview

The post holder will work as a specialist practitioner within the Dudley Heart Failure Team both in the acute trust and community setting, assisting in the provision of a comprehensive service to an allocated caseload of patients with Acute or Chronic Heart Failure. The service operates from 08:00 to 16:00, seven days a week, including bank holidays. The aim is to reduce patient length of stay, hospital admissions and readmissions, and to support early discharge by expediting safe and timely proactive care.

The post holder will deliver comprehensive clinical assessment and care aimed at improving quality of life, supporting and empowering patients to manage their condition, and enhancing the end‑of‑life experience for patients and their carers. The post holder will also ensure the service reflects the Trust’s KPIs and will support the Heart Failure Lead and Matron in achieving these KPIs.

• Work as a specialist practitioner within the Dudley Heart Failure Team, assisting in the provision of a comprehensive service to an allocated caseload of patients with Chronic Heart Failure, including in‑reaching into hospital to provide a seamless service.
• Provide service seven days a week, including bank holidays, aiming to facilitate a reduction in patient length of stay and hospital admissions/readmissions, and to assist early discharge by expediting safe and timely proactive support.
• Deliver comprehensive clinical assessment and care aimed at improving quality of life, supporting and empowering patients to manage their condition, and improving the end‑of‑life experience for patients and carers.
