Heart Failure Specialist Nurse; Virtual Ward

Job Overview

An exciting band 6 opportunity for registered nurses who have experience in cardiology has arisen in our heart failure virtual ward which is part of Dudley Heart Failure Team. The successful applicant will be responsible for the management of patients with heart failure over a 7 day service. This may also include in-reaching into hospital in order to provide a seamless service.

• Provide direct face-to-face and virtual monitoring to deliver acute patient management with a confirmed heart failure diagnosis in the community setting rather than hospital.
• Manage patients admitted onto the virtual ward on an individual basis for up to a 14‑day period.
• Work as a specialist practitioner within the Dudley Heart Failure Team Virtual Ward, assisting in the provision of a comprehensive service to an allocated caseload of patients with chronic heart failure, including in‑reaching into hospital and out‑reaching into the community to create a seamless service.
• Deliver comprehensive clinical assessment and care aimed at improving quality of life, supporting and empowering patients and their carers to manage their condition.
• Operate the service 7 days a week including bank holidays, with the aim of reducing patient length of stay, hospital admissions/readmissions and facilitating early discharge from hospital.
