ABL Health delivers the Integrated Health Improvement Service, Your Health Dudley, commissioned by the Dudley Council Public Health Team. This transformative service will play a central role in improving the health and well‑being of residents of Dudley. The service provides integrated behaviour support to help people move more, drink less alcohol, stop smoking and lose weight. The service also offers lifestyle support for families, delivers community health checks, offers advice and guidance, and proactively signposts to relevant organisations.
All of this is embedded in sustainable behaviour change and positive well‑being. This post will focus on the Healthy Lifestyle support for families with children aged 4-17.
As a Family Healthy Lifestyle Practitioner, you will work closely with children, young people and families to improve health and wellbeing through personalised support and evidence‑based behaviour change interventions. You will provide one‑to‑one support via face‑to‑face appointments, telephone and video consultations, helping families to set achievable SMART goals and sustain healthy lifestyle changes. Working collaboratively with schools, community organisations, commissioners and local partner agencies, you will build strong referral pathways, promote the service, and increase engagement while reducing drop‑out rates.
You will design and deliver engaging workshops on topics such as healthy eating and active lifestyles, contribute to the development of family‑friendly resources and social media content, and gather client feedback, case studies and testimonials to support service improvement. The role also involves maintaining accurate records and performance data, contributing to KPI achievement, safeguarding children and families in line with organisational policies, supporting the wider integrated service, attending meetings and training, and working flexibly to meet the evolving needs of the service.

Job responsibilities- Increase new referrals to the service and reduce drop‑out rates
- Provide one to one family support, via face‑to‑face sessions, phone or video calls
- Work with local schools to encourage parents to engage with the service
- Build on network of signposting opportunities to enrich support offered
- Build local links with other support service staff to encourage referrals and partnership working
- Promote the service at local meetings, networks and events
- Work with Service Manager and Co‑ordinator to ensure service is achieving KPIs
- Work with data team to ensure system reporting meets the needs of the service
- Attend ABL forums, meetings and training as applicable
- Work with marketing to create family friendly booklet material and social media posts
- Work with system in order to extract data for use in reports and at managers request from ABL meetings and updates
- Maintain good working relationship with Family Commissioners
- Co‑create and deliver healthy workshops for use with families, such as the Healthy Lunch box session
- Group workshop delivery out in the community, such as healthy eating workshops, healthy pizza making and delivering fun and informative lifestyle topics
- Contribute to collecting feedback by collecting regular case studies and client testimonials
- Support individuals and families to set SMART goals and offer support using evidence‑based behaviour change techniques
- Record data accurately on database, keeping records up to date and in line with policy
- Safeguard all families and report in line with ABLs safeguarding policy
- Support the wider team in delivering the Integrated Service
- Adhere to all ABLs policies and procedures
- Carry out any other duties as requested by the Head of Service
