IDA Conference in Gelderland zoekt een Klantcoördinator Zorg om de perfecte match tussen cliënt en zorg te maken. In deze rol coördineer je passende zorg, onderhoud je een netwerk en stel je zorgplannen op voor klanten. Je werkt binnen een warm team en krijgt de ruimte voor ontwikkeling en innovatie.
Je hebt een afgeronde HBO-zorg gerelateerde opleiding en ervaring in de zorg, bij voorkeur als casemanager of verpleegkundige. Kennis van wet- en regelgeving is vereist en je moet bij voorkeur in de regio Liemers wonen.
