Position Overview
Center Well Home Health is seeking a Baylor Registered Nurse to deliver personalized, one‑on‑one care to patients in the comfort of their homes. Working closely with a dedicated team of physicians and clinicians, you will develop and manage care plans that support recovery and help patients regain independence.
Key Responsibilities- Provide admission, case management, and follow‑up skilled nursing visits for home health patients.
- Administer ongoing care and case management for each patient, providing necessary follow‑up as directed by the Clinical Manager.
- Confer with the physician to develop the initial plan of treatment based on physician orders and initial patient assessment, and revise the plan in consultation with the physician as required.
- Coordinate appropriate care, encompassing various healthcare personnel (Physical Therapists, Occupational Therapists, Home Health Aides and external providers).
- Report patient care, condition, and progress to the patient’s physician and Clinical Manager on a continuous basis.
- Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs and problems.
- Coordinate, oversee, and supervise the work of Home Health Aides, Certified Home Health Aides and Personal Care Workers while monitoring appropriate documentation completion.
- Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.
- Prepare appropriate medical documentation on all patients, including case conferences, patient contacts, medication order changes, re‑certifications, progress updates, and care plan changes.
- Diploma, Associate or Bachelor Degree in Nursing.
- Minimum of one year nursing experience preferred.
- Strong medical‑surgical, ICU, ER, acute experience.
- Home Health experience a plus.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Strong organizational and communication skills.
- Valid driver’s license, auto insurance and reliable transportation.
$70,500.00/yr – $96,900.00/yr
