Job Summary

The Housing Specialist will provide comprehensive housing services to Positive Impact Health Centers (PIHC) clients who receive housing assistance via the agency’s housing programs. The Housing Specialist’s role includes developing a housing plan to address barriers to stable housing; connecting the client with medical care to address HIV; and coordinating supportive services that help the client reach identified goals and achieve self-sufficiency.

Responsibilities
  • Develop a housing procurement, financial and self-sufficiency case management plan with clients seeking assistance in the Housing Program.
  • Assist in the development of and encourage adherence to a personal budget and budget counseling sessions; provide budget counseling and education to assist clients in establishing payment plans for past due bills and debts.
  • Take initiative in problem-solving and utilize resources, including with team members and community resources, to assist in addressing challenges.
  • Ensure that client records are maintained in compliance with Federal, State, and other grant requirements.
  • Work in interdisciplinary teams to provide wrap-around services to program participants.
  • Assist participants in the development of a strength-based/solution-focused individualized housing goal and action plan that promotes self-sufficiency.
  • Work closely with community agencies and services to develop partnerships that will offer life skill training (e.g. employment training, financial literacy, job placement) for program clients.
  • Act as a patient advocate and liaison between the client and community service agencies.
  • Assess housing-related requests efficiently.
  • Ensure that program deliverables are met or exceeded during the project period.
  • Ensure that client records are maintained in compliance with Federal and other grant requirements.
  • Monitor housing project goals and work to achieve housing project deliverables.
  • Assist with completing project related reports and audits as needed.
  • Document all client encounters appropriately and in a timely manner into EMR systems.
  • Attend regular PIHC staff meetings, trainings, and other meetings as requested.
  • Participate in face-to-face meetings with assigned clients as indicated according to the guidelines of each housing project.
  • Field high volume of housing-related calls.
  • Attend project-related training sessions, meetings and conferences as assigned.
  • Attends weekly team meetings.
  • Complies with PIHC's policies and procedures.
  • Connect PIHC clients with necessary resources including, but not limited to housing resources, mental health services, medical care, food stamps, educational programs, employment resources, and substance abuse treatment.
  • Document PIHC housing clients’ service plan goals as directed by housing project guidelines.
  • Communicate with landlords regarding PIHC housing programs. Contact private and corporate landlords to develop collaborative partnerships for low-income housing candidates.
  • Maintain and update all housing-related spreadsheets.
  • Traveling to different sites to complete housing intakes.
  • Other duties as assigned.
Minimum Qualifications
  • Bachelor’s degree in social service specialty (e.g. social work, psychology, sociology, public health). Experience in housing, social work, or case management is highly preferred.
