  Schedule:
  
  Generally 24-40 hours per week with a flexible Monday through Friday schedule. Hours can be customized to fit what works best for you.
• Settings:
  Primarily treat patients in SNF/LTC and home health. Potential for Site Coordinator leadership. Joining a team with an excellent work culture and strong longevity.

• Provide physical therapy services to patients in SNF/LTC and home health settings.
• Collaborate with a multidisciplinary team to deliver high-quality patient care.
• Potential to assume Site Coordinator leadership responsibilities as the role expands.

• Graduated from an accredited Physical Therapist program.
• Eligible for, or currently have, PT licensure in Minnesota.
