Position Summary:
We?re looking for a Physician Assistant (PA-C) who believes in person-centered, place-based care
. Instead of a rushed clinic setting, you?ll have time to connect with patients in their own environment?where your care can truly change outcomes.

Responsibilities:

• Provide routine and follow-up primary care to older adults
• Collaborate with care teams and support staff on treatment planning
• Monitor chronic conditions and acute medical concerns
• Use EMR to document care clearly and consistently

Qualifications:

• MN PA license in good standing
• Experience in geriatrics, care transitions, or chronic disease management
• Compassionate and collaborative
• Valid driver?s license and auto insurance
