We are looking for a Physical Therapist (PT) to assess, teach, and deliver therapy interventions directed by a physician and established in the home health plan of care. You will collaborate with interdisciplinary team members, sharing updates to optimize patient well-being.

• Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current level of function.
• Administer, evaluate, and interpret tests and measurements to determine functional goals and outcomes.
• Prepare written evaluations and plans of care defining PT needs, goals, progress, and expected outcomes.
• Deliver physical therapy services in compliance with prescribed physician orders.
• Maintain the plan of care through timely documentation, modifying goals and treatments as needed, and determine discharge care plans.
• Manage discharge planning, including collaborating and educating patient and caretakers on home exercise plans.
• Coordinate services with healthcare team, physicians, parent/guardians, nurses, etc.
• Document according to policy and procedure with minimal supervision or instruction; avoid technical or clinical denials.
• Communicate effectively with staff, healthcare team members, and family; serve as a resource consultant for clinical staff.
• Supervise other personnel (PTA, HHA, OTA) as applicable.
• Complete OASIS assessments where allowed by state regulations.
• Adhere to all patient care standards, licensure regulations, and state laws.

• Degree in Physical Therapy from an accredited university or program.
• Current, unrestricted state license as a Physical Therapist in the state of practice.
• Demonstrated proficiency in clinical assessments, documentation, and compliance with policies and procedures.
