Center Well Home Health is seeking a Home Health Occupational Therapist to help patients regain independence in their homes. You will assess, plan, and implement therapeutic interventions to improve function and quality of life, coordinating with physicians and the care team.
This role requires current OT licensure and CPR, a valid driver's license, and reliable transportation. You will work within an interdisciplinary team to support rehabilitation, patient education and successful discharge
