Cornerstone is seeking Complex Care Practitioners to join our team in a permanent role, offering between 16 and 39 hours per week with additional relief opportunities for flexibility. You’ll support individuals with complex care needs in their flats, coordinating with families and a multi-disciplinary team to implement robust, outcome-focused care plans.
You’ll help people maintain independence, engage in meaningful activities, and access community leisure.
