As a Case Worker, you will actively engage in comprehensive assessments and develop individualized support plans for residents facing various life challenges.

Key Responsibilities:
• Document and implement personal service plans
• Evaluate client needs and create short-term solutions
• Oversee emergency responses and safety measures
• Assist in community-based activities and transportation
• Maintain thorough documentation on client progress
Requirements:
• Diploma in human/social services or similar
• One year experience with at-risk populations
• Excellent verbal and written communication
• Successful Criminal Records Check mandatory
• Up-to-date First Aid Certification is a plus
