Position: Clinical Pharmacist Role at Care Bridge Clinic
Advance community health as a Clinical Pharmacist at the Care Bridge Clinic, Island Health in Duncan, BC. This full-time position calls for expertise in medication management within a dynamic primary care environment.

You will play a key role in an inter-professional team delivering clinical pharmacy services, addressing medication-related issues, and performing patient assessments. The position involves traveling to various sites as necessary and requires a commitment to effective patient communication and safety. A valid Class 5 Driver's License is essential.

Key Responsibilities:

• Provide essential clinical pharmacy services across sites

• Identify and manage patient drug therapy issues

• Conduct patient assessments and deliver medication education

• Collaborate effectively within an inter-disciplinary team

• Ensure compliance with safety protocols during travel

Requirements:

• Licensed by the College of Pharmacists of British Columbia

• At least two years of direct patient care experience

• Valid Class 5 Driver's License for site travel

• Strong problem-solving and critical thinking capabilities

• Familiarity with Electronic Health Records systems
