Key Responsibilities:

• Assess and manage clients across various care settings

• Provide health education for chronic disease self-management

• Advocate for culturally respectful care and community connections

• Facilitate client transitions across healthcare services

• Collaborate with the Primary Care team and external health services

Requirements:

• Registration with BC College of Nurses

• Two years of recent nursing experience including primary care

• Valid BC driver's license

• Basic Life Support, Level C certification

• Strong communication and time management skills
