As a Community Inclusion Worker, your role is to assist and empower individuals in pursuing their personal goals within the community. You will coordinate various activities that foster inclusion and are tailored to the individual’s preferences. Building relationships with families, professionals, and community organizations is vital for a holistic approach to support.

Key Responsibilities:

• Advocate for rights and choices of each person

• Facilitate community-based activities for supported individuals

• Document daily interactions and health details accurately

• Collaborate on Individual Service Plans for effective support

• Apply behaviour management strategies appropriately

Requirements:

• High school diploma or Community Support Worker Diploma

• At least 2 years in relevant social services work

• Valid driver’s license and reliable vehicle

• Current First Aid/CPR certification

• Criminal record clearance for working with vulnerable populations
