Community Caseworker

Position: Community Caseworker at Connective
Join Connective as a Community Caseworker, providing essential support in a non-profit setting. Enhance residents' quality of life through individualized care plans and community resources.

As part of Connective’s team, you will deliver casework and program support to enhance the lives of residents. Your work will focus on developing personalized service plans, coordinating with other professionals, and promoting independent living skills. This role requires a compassionate approach towards individuals facing homelessness, mental health challenges, and other barriers to stable housing.

Key Responsibilities:

• Develop and document personal service plans for residents

• Assist with daily living skills to promote independence

• Evaluate and adjust support plans based on resident needs

• Administer medication following established protocols

• Liaise with community resources and support services

Requirements:

• Diploma in social service field with one-year experience

• Experience working with marginalized populations

• Ability to respond effectively in crisis situations

• Current First Aid Certificate and Criminal Records Check

• Open to completing Food Safe Certificate
