Licensed Clinical Social Worker, Case Management
DRH Health is seeking a Licensed Clinical Social Worker (LCSW) to join our Case Management team. The Social Worker will perform psycho‑social assessments, coordinate care, and provide direct interventions with patients, families, and interdisciplinary teams across the continuum of care.
Responsibilities- Conduct comprehensive assessments of patients’ health, functional, behavioral, psychological, and social needs.
- Identify patient discharge and transition needs and collaborate with physicians, allied health, and case managers to develop care plans.
- Educate patients, families, and team members on treatment options, insurance benefits, psychosocial and financial concerns.
- Coordinate referrals to specialty and community‑based services and advocate for patients’ needs.
- Provide crisis intervention, short‑term counseling, and bereavement support as needed.
- Investigate child or elder abuse or neglect cases and take authorized protective action when necessary.
- Maintain confidential patient records and safeguard information.
- Participate in interdisciplinary team meetings and communicate case progress.
- Continuously evaluate case‑management plans and apply outcome measures aligned with regulatory standards.
- Adhere to safety protocols, including respirator fit testing and required vaccinations.
- Perform other related duties as assigned.
- Bachelor of Social Work (BSW) accredited by CSWE;
Licensed Social Work Associate (LSWA) preferred. - Master’s degree in Social Work (MSW) or Licensed Clinical Social Worker (LCSW) preferred.
- Previous experience in a healthcare setting preferred.
- Excellent written, verbal, and interpersonal communication skills.
- Proficiency in electronic health records and basic computer skills.
- Valid Oklahoma state license in good standing; current driver’s license and automobile liability insurance required.
- Ability to adapt to urgent situations and varying developmental needs.
This is a “Safety‑Sensitive” position as defined by Oklahoma law. Applicants must not test positive for THC in any pre‑employment or post‑employment drug test. Respirator fit testing and required vaccinations are mandatory.
