- Provide primary care rounding in post-acute and skilled nursing facilities - Manage chronic disease, perform diagnostic evaluations, and create individualized care plans - Collaborate closely with NPs, facility leadership, and interdisciplinary teams - Build long-term relationships with patients, families, and staff - Mentor advanced practice providers and participate in quality improvement initiatives :

Board Certified / Board Eligible in Family Medicine or Internal Medicine Active and unrestricted Texas medical license and DEA registration Minimum 1-2 years of post-acute or primary care experience Strong interpersonal and organizational skills with a patient-centered approach & Benefits:
