Trainee Assistant Practitioner; TAP at Family Nursing and Home Care

This is an exciting opportunity to join FNHC within our forward-thinking professional Rapid, Reablement, Response Team (RRRT).

You will act as an assistant practitioner to effectively support registered staff with care planning and to deliver and evaluate care for a specified group of patients/clients with a range of conditions within a recognised clinical area. You will be required to work at an advanced non-registered level within a community-based setting, holding individual accountability for your practice.

Essentially you will have minimum 3 years as a SHCA and hold Level 3 RQF or equivalent. As a trainee you will be willing to complete the Assistant Practitioner foundation degree (therapy) and meet the competencies required within 2 years of starting the programme.
