Social Worker Adult Integrated Community Care Team
Sanctuary Personnel, an innovative recruitment agency has a new permanent position for a Social Worker within the Integrated Adult Community Care Team, working full time based in Guernsey.
The salary for this permanent Social Worker job is up to £62,310 per annum.
- Provide a social work service to adults living in the Bailiwick of Guernsey, carrying out assessments using the agreed standardised format and allowing adults to remain in their own homes where possible.
- Form relationships with service users to produce accurate assessments of complex psychosocial needs and assess for services in line with local policy.
- Provide complex psychosocial support for service users with terminal or life‑threatening illness and their families, advocating for all.
- Take a key role in responding to referrals to the Adult Social Work Team and carry a caseload of adults in need.
- Assess service users, carers and family’s psychosocial concerns, considering cultural, spiritual, social and financial issues and evaluating risk in partnership with other multi‑professional team members.
- Use expert social work skills to assess family’s emotional resilience, coping skills, strengths and difficulties; discuss with the team as appropriate.
- Formulate appropriate interventions with the multi‑professional team, personal networks and community resources.
- Undertake person‑centred assessments, evaluate & review needs regularly and readjust care plans when necessary.
- Attend and contribute to formal multi‑professional meetings to review and plan all aspects of service users’ care, chairing when appropriate.
- Develop effective links with families and carers to achieve comprehensive assessment and care planning.
- Demonstrate a high standard of person‑centred practice, reflecting a value‑based approach to working with adults and families.
Requirements of this Social Worker role:
- A Social Work Qualification or equivalent.
- Understanding of relevant legislation.
- Relevant Social Work experience.
