Clinical Lead – Registered Nurse

.

Working closely with the Home Manager, you will take a lead role in supporting high standards of clinical practice across the service. Alongside providing direct nursing care and clinical cover, you will provide clinical leadership to the wider care team, support safe and effective clinical decision-making and help ensure care is delivered consistently in line with professional, regulatory and Company standards.

You will have a particular focus on clinical standards, medication management, care planning, risk management, clinical audits and quality improvement, providing the Home Manager with professional nursing support and helping to maintain a safe, effective and person-centred service.

• Provide clinical leadership and professional nursing support across the service, working closely with the Home Manager.
• Provide direct nursing care and clinical cover as required, including leading shifts and responding to changing clinical needs.
• Assess, plan, deliver and evaluate high-quality, person-centred nursing care.
• Support and monitor high standards of clinical practice, identifying areas for improvement and escalating concerns appropriately.
• Take a lead role in medication management, including safe administration, medication audits and supporting actions arising from identified issues.
• Support the quality and effectiveness of clinical assessments, care plans, reviews and risk assessments.
• Undertake and contribute to clinical audits, identifying areas for improvement and supporting the implementation and monitoring of action plans.
• Monitor the health and wellbeing of the people we support, recognising signs of deterioration and ensuring appropriate clinical action and escalation.
• Review and contribute to learning from clinical incidents, medication errors, falls and other relevant clinical events.
• Provide clinical guidance, support and supervision to care staff and support the assessment and maintenance of relevant clinical competencies.
• Work collaboratively with the people we support, their families, GPs, Advanced Nurse Practitioners, district nurses, pharmacy teams, paramedics and other healthcare professionals to coordinate care.
• Maintain accurate care records, medication records and clinical documentation.
• Promote safeguarding, infection prevention and control, safe clinical practice and effective risk management.
• Support the Home Manager in maintaining clinical compliance and inspection readiness.
• Promote continuous improvement and contribute to the development of high standards of care across the service.
• Ensure your own practice and the clinical standards you promote are consistent with NMC standards, Health & Social Care Standards, Care Inspectorate requirements and Company policies.

• Current NMC registration with an active Nursing PIN.
• Previous experience as a Registered Nurse, ideally within a care home or similar care setting.
• Strong clinical assessment, medication management and clinical decision-making skills.
• Experience of assessing needs, developing person-centred care plans and reviewing outcomes.
• Experience of clinical audits, quality improvement or supporting clinical governance would be advantageous.
• The confidence and experience to provide clinical leadership and guidance to the wider care team.
• Strong communication, leadership and organisational skills.
• Ability to work collaboratively with the Home Manager and other healthcare professionals.
• A compassionate, professional and person-centred approach to care.
• A commitment to continuous professional development and maintaining high clinical standards.

Previous experience in a senior nursing or clinical leadership role, dementia care, mentoring colleagues or undertaking additional clinical training would be advantageous but is not essential.
