An exciting opportunity has arisen for a dynamic and innovative nurse to join NHS Fife’s Community Cardiac Specialist Nurse Team as a Heart Failure (HF) Advanced Clinical Nurse Specialist (ACNS).

The HF CNS Service operates Monday to Friday during business hours. However, a new model of a 7‑day working week may be implemented in the future.

We provide holistic and evidence‑based specialist care to people diagnosed with HF across Fife. We have two bases:
Stratheden hospital and Lynebank hospital. The post holder will participate in the ongoing development, implementation and evaluation of the HF clinical service, improving the management, treatment and support of patients with HF and their carers/families.

This is a specialist, autonomous role involving advanced assessment, medication optimisation, patient education, and close collaboration with cardiology, primary care and community teams.

You will manage your own caseload, contribute to ongoing service development and play a key role in preventing avoidable hospital admissions through proactive, patient‑centred care. The successful candidate will demonstrate strong clinical judgement, excellent communication skills and a passion for improving outcomes for people with long‑term cardiac conditions.

The successful candidate will be offered a band 7 if:

• You are an experienced HF nurse specialist with a non‑medical prescribing qualification and have undertaken Level 11 post‑graduate qualifications in clinical assessment & decision making and chronic heart failure.
• Your role will involve supporting the Team Leader under the leadership pillar, developing the service and deputising for the Team Leader when required. Previous leadership experience is desirable but not essential, as training will be provided.

The successful candidate will be offered the trainee post under annex 21 if:

• You are an experienced nurse with significant cardiology experience, educated to degree level with evidence of recent CPD, and willing to undertake the qualifications listed above. This training will be conducted under Annex 21 (see attached for more information).

You will require excellent communication skills and be able to work as part of the wider Health and Social Care Team with the ability to work autonomously, utilising your own initiative, clinical judgement and decision making.

As this is a Fife‑wide community post you will be required to reliably and efficiently travel when necessary across various sites, bases and patients' homes throughout Fife.

A requirement of this post is to become a member of the Protection of Vulnerable Groups (PVG) Scheme prior to appointment.
