NHS Fife is seeking a Heart Failure Advanced Clinical Nurse Specialist to join the Community Cardiac Specialist Nurse Team. The post focuses on autonomous clinical practice, advanced assessment and medication optimisation to support patients with long-term cardiac conditions.
You will educate patients and carers, work with cardiology and primary care teams, manage your own caseload, and contribute to service development while travelling across Fife bases as required.
