Behavioral Health Connections (BHC) in Dunwoody, GA is seeking a dynamic and talented LPN for Referral Management (Weekday varied shift, Full Time, 10‑hour shifts) to review assessment referrals and work closely with the RN and other clinicians to determine the appropriate placement of patients and aid the call center specialist with duties to enhance referral management process.

• Review assessment referrals received and collaborate with RN to assess patient needs.
• Assist with coordination of patient care.
• Maintain patient records, update databases, and ensure accurate documentation of patient information.

Education Nursing Degree, preferred Bachelors or Masters.

Licensure/Certification Licensed LPN in current State or compact license.

Current CPR, if required by a customer hospital.

Experience Preferred 1-3 years related work experience in Behavioral Health.
