Careage Home Health is seeking a Home Health Physical Therapist to join our team serving Pierce County
. This position is a great opportunity to grow your PT skillset while providing essential one-on-one care to individuals in the home care setting.
We offer a competitive hourly rate for all patient visits, great benefits and travel time, and this position will travel throughout Pierce County.

Summary of Responsibilities- Travel within Thurston County to provide care on an intermittent basis in patients' homes.
- Provide physician orders and plan of care under the direction and supervision of the Director of Clinical Services.
- Work in conjunction with all other home care personnel in patient care planning.
- Direct and supervise Physical Therapy Assistant and Home Health Aide, as assigned.
- Take initial patient history and make initial evaluation.
- Consult with physicians regarding change in treatment.
- Instruct patients and family in home programs and activities of daily living.
- Participates in Quality Assessment and Performance Improvement activities as assigned.
- Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition.
- Appropriately utilizes ICD-10 codes.
- Graduate from a physical therapist curriculum approved by the American Physical Therapy Association.
- Currently licensed as a physical therapist in the state of Washington.
- Two (2) years' experience as a physical therapist, preferred.
- Home Health experience, preferred.
- Detail oriented, self motivated and capable of efficient time management.
- Critical thinker capable of working collaboratively with a team.
- Valid WA State driver's license.
