Lykos Medical is seeking an experienced and compassionate Nurse Practitioner or Physician Assistant to provide mobile primary care services to seniors residing in assisted living communities across Colorado
. This role is ideal for clinicians who are ready to step away from the high-volume clinic or hospital setting and deliver deeply personalized care, one patient at a time.

You’ll be a key player in a collaborative, mission‑driven team that prioritizes connection, trust, and dignity in every patient interaction.

• Provide comprehensive in‑person primary and geriatric care in assisted‑living communities
• Manage chronic conditions, conduct wellness and cognitive assessments, and provide urgent visits
• Collaborate with caregivers, families, and other healthcare providers for coordinated care
• Participate in transitional care planning and end‑of‑life support
• Accurately document visits using EMR and contribute to quality improvement efforts
• Serve as a clinical leader and advocate for best practices in senior care

• Licensed Nurse Practitioner or Physician Assistant in the state of Colorado
• 3+ years of clinical experience in internal medicine, geriatrics, or hospitalist settings required
• Strong ability to work independently in field‑based environments
• Excellent clinical judgment, communication, and EMR documentation skills
• Valid Colorado driver’s license and reliable transportation
