Program

This position is for the Back Home SSVF (Supportive Services for Veteran Families) program – a Rapid Rehousing program using a 90‑day model to find and stabilize housing for veterans experiencing homelessness or  role focuses on housing case management and wrap‑around services to connect veterans to public benefits, VA benefits, health referrals, and financial health referrals.

Job Summary

The Case Manager directs and facilitates the delivery of support services to participant households, conducting needs‑based eligibility assessments, engaging in community outreach, providing case management services, and coordinating program‑wide services.

Essential Duties and Responsibilities
  • Provides case management services to program clients.
  • Identifies and serves the unique needs of participant households and uses screening and assessment tools to ensure completion of needs‑based screenings.
  • Implements the program in accordance with funder and organization standards.
  • Conducts community outreach to identify eligible households with barriers to housing stability.
  • Provides group educational and other activities for current or prospective program clients as assigned.
  • Serves as a resource for team members on best practices and current field issues.
  • Attends orientations, trainings, education programs, staff meetings, community meetings, conferences, and workshops as requested.
  • May provide training and mentorship to team members and community stakeholders.
  • Documents all client interactions and required eligibility information using designated databases.
  • Maintains confidentiality and professionalism in all interactions.
  • Participates in professional development activities to enhance knowledge of best practices.
  • Performs all other duties as assigned.

Requirements

Minimum Qualifications
  • Bachelor’s degree in human services, social work, or related field, or related experience.
  • One year of direct experience in human services.
  • Colorado driver’s license and state‑mandated auto insurance.
  • Personal vehicle available for work‑related travel (mileage reimbursement).
  • Complete agency and program credentialing within 12 weeks of hire and maintain standards thereafter.
Preferred Qualifications
  • SOAR (SSI/SSDI Outreach, Access, and Recovery) Certification.
Knowledge and Skills
  • Knowledge of Harm Reduction, Critical Time Intervention, Motivational Interviewing, Trauma‑Informed Care, and Housing First Principles.
  • Ability to serve individuals from diverse backgrounds respectfully and professionally.
  • Ability to work independently and within a multicultural team.
  • Effective verbal and written communication.
  • Self‑care skills for dealing with difficult or traumatic situations.
Working Conditions and

Physical Requirements
  • Regular travel within service area.
  • Work in diverse environments (shelters, streets, offices, etc.).
  • Physical tasks include bending, climbing, stooping, kneeling, reaching, squatting, lifting 30‑50 lbs, standing, walking, etc.
Position Type and Hours
  • Full‑time; 40‑hour workweek with occasional overtime.
  • Potential for hybrid work.
Compensation

Salary range: $22.00 – $24.00 per hour (based on experience).
