Lighthouse Behavioral Wellness Centers is seeking a dedicated Care Coordinator to support individuals and families navigating mental illness, trauma and addiction. You will coordinate care across home, school and community settings, integrating family strengths and cultural needs into treatment plans.
You will conduct assessments, develop and monitor care plans, coordinate with internal and external providers, and ensure documentation meets ODMHSAS standards while promoting safety and recovery.
