We are looking for a Case Manager to join a healthcare team supporting inpatient mental health and rehabilitation programs in the Upper Highway area. Experience in hospital or inpatient mental health settings is essential. This role focuses on coordinating patient care, developing individualized treatment plans, and managing multidisciplinary teams to ensure safe, effective rehabilitation and recovery.

• Conduct intake assessments and verify referral information
• Develop and adjust individualized treatment plans
• Coordinate psychiatry, psychology, nursing, OT, and social work teams
• Facilitate MDT meetings and track patient progress
• Liaise with families, employers, and referring practitioners (with consent)
• Lead discharge planning and aftercare coordination
• Monitor clinical risks and advocate for patients
• Maintain accurate, confidential records in compliance with MHCA, POPIA, and professional standards

• Registered Nurse (SANC), Social Worker (SACSSP), or Occupational Therapist / Psychologist / Registered Counsellor (HPCSA) with active registration
• Minimum 4 years’ experience in inpatient mental health, psychiatric, or rehabilitation settings
• Knowledge of multidisciplinary treatment models and mental health legislation
• High emotional intelligence and strong communication skills
