About the Role

Our client seeks a dedicated Social Work Case Manager to join their team in Durban . This role involves providing essential support and guidance to vulnerable individuals and families, with a blend of remote and in-person client interactions. You will assess needs, develop intervention plans, and connect clients with necessary community resources to improve their well-being and stability. • Conduct comprehensive assessments of client needs, both remotely and through in-person visits.
• Develop, implement, and monitor individualized case management plans.
• Connect clients with appropriate social services, healthcare providers, and community support networks.
• Provide counseling and crisis intervention support as needed.
• Maintain accurate and detailed case notes and progress reports in the electronic system.
• Collaborate with other professionals and agencies to ensure coordinated care for clients.

• Registered Social Worker with current professional body registration.
• Minimum of 3 years of experience in case management or social work.
• Strong knowledge of social welfare services and resources available in Durban .
• Excellent interpersonal, communication, and active listening skills.
• Proficiency in using case management software and virtual communication tools.
• Ability to work independently and manage time effectively in a hybrid work environment.
