Outpatient LPN: Diabetes Care & Education

The Hospital of Central Connecticut, part of Hartford Health Care, is seeking an LPN to provide direct care under the supervision of a Registered Nurse in outpatient and non-acute settings such as Endocrine Clinic and Diabetes Education.

You will collect health information, assist in carrying out the physician’s plan of care, educate patients, and coordinate care with the health care team while following Nursing Department policies.
