The Patient Revenue Management Organization (PRMO) is Duke Health’s fully integrated, centralized revenue‑cycle organization that supports the entire health system in streamlining scheduling, registration, coding, billing, and other essential revenue functions.

Role Overview

The Transplant Financial Coordinator (TFC) is part of a multidisciplinary team charged with providing accurate and professional financial clearance to patients seeking Transplant Services  role involves verifying accurate demographic and financial information, reviewing and interpreting insurance benefits, performing financial counseling and screening, establishing financial arrangements, and coordinating referrals to support patients with discharge and post‑hospital care. The TFC works closely with Utilization Review, Transplant Billing, and clinical teams to ensure correct and timely reimbursement.

The position operates on a hybrid schedule Monday through Friday 8:00 am–4:30 pm, with on‑call duties after hours and on rotating weekends and holidays.

Primary Duties and Responsibilities
  • Detail patient insurance benefit information for all aspects of the transplant process including outpatient prescriptions, travel and lodging, studies, and fundraising.
  • Review benefits and other transplant financial issues with patients and family members during transplant evaluation.
  • Obtain payer authorizations for transplant evaluation and surgical/wait‑list status.
  • Coordinate communication between insurance companies, governmental agencies, providers and internal departments.
  • Work with DUHS Finance, Managed Care Contracting, Pharmacy assistance programs to develop agreements for international, self‑funded patients and patients with limited benefits.
  • Interact with Transplant and Oncology clinical teams to obtain treatment plans, discharge planning, and demographic data for insurance verification.
  • Interview admitted patients to obtain required information to begin verification.
  • Interview patients prior to financial clearance, providing key benefit information and data to support clinical teams.
  • Prepare financial packets based on each patient’s benefits, interview, and estimated expenses.
  • Share financial screen daily/weekly with clinical teams at their respective meetings.
  • Notify patients if not financially approved for evaluation at Duke and redirect them to the insurance payer case manager.
Data Maintenance and Billing Information (Transplant Specific)
  • Facilitate resolution of transplant patient billing issues.
  • Use clinical and billing knowledge to identify and resolve billing errors/inconsistencies.
  • Understand federal and state guidelines related to organ transplant.
  • Accurately identify the guarantor account for the visit in Maestro.
  • Update demographic and insurance information in Maestro and the transplant database when changes occur.
  • Review each patient visit to determine transplant‑related status for accurate billing.
  • Confirm daily that visit requirements are met through clinician communication.
  • Continuously analyze patients’ eligibility, benefit changes, and financial status through transplantation.
  • Provide weekend and evening coverage for solid organ transplant referrals to supply timely financial feedback to surgeons.
Patient Identification / Pre‑registration
  • Update demographic and insurance information in Maestro Care.
  • Obtain a Duke Medical Record number for new patients.
  • Analyze benefits for appropriate coverage, review contracts, and obtain clarification from Managed Care Contracting and Payor Coordination as needed.
  • Resolve Maestro Care alerts for missing or incorrect registration and insurance elements.
  • Add or edit insurance per policy and procedure.
  • Clear TES edits and hold bills.
