Job Description Overview
We are seeking a results-oriented, dynamic, and dedicated Qualified Professional to join our growing team. This role involves providing therapeutic support to residents in a group home setting. The position offers an exciting opportunity to make a meaningful difference in the lives of children and adults with disabilities and their families through evidence-based practices. The role provides clinical and administrative support, including client care management, staff supervision and training, treatment planning and coordination, quality assurance, and compliance.

Job Description
- Supervision of its associate professional(s) and direct care professionals.
- Oversight of emergencies.
- Provision of direct psycho educational services to clients.
- Interview clients and their representatives to determine preferences and needs.
- Conduct and review assessments.
- Develop individualized treatment plans for clients and communicate the plan to staff.
- Maintain accurate documentation in compliance with healthcare regulations.
- Collaborate with interdisciplinary teams to enhance client care.
- Monitor client progress and adjust treatment plans accordingly.
- Assist with teaching appropriate social and independent living skills.
- Encourage program persons to perform tasks as independently as possible, assisting them to participate in all activities to the maximum extent possible.
- Accurately document provision of care as directed.
- Stay current with changes in care/support (e.g. medications, treatments, behavior plans, etc.).
- Alert program management in the event of a person’s change in status or behavior.
- Complete all documentation and Incident Reports within program timelines.
- Actively participates in person, family, team, and staff meetings.
- A bachelor's degree in a Human Service field with minimum of two years proven experience working with IDD population.
- Experience in group home settings is preferred.
- Familiarity with documentation in clients chart.
- Working knowledge of applicable practice regulations
- Valid driver’s license, auto insurance, and reliable transportation
- Ability to pass background check
