Duke Health's Home Care & Hospice division is seeking a Care Coordinator in Durham, NC. The role focuses on coordinating home health, hospice, and infusion services for patients at Duke University Hospital with occasional rotations to Duke Regional Hospital.
You will educate patients and families and ensure safe, seamless care transitions, including documentation and interdepartmental collaboration. The schedule is Monday–Friday, 8:30 AM–5:00 PM, with an on-call rotation every 4–6 weeks.
