Position: SSVF Carolinas Case Manager (63374)

Job Summary

Provide day‑to‑day support and services for veteran households. Bring relevant experience in homelessness prevention, rapid re‑housing, and transitional supportive services. Act as a key link from intake to permanent housing, promoting outreach, case management, advocacy, and wrap‑around supports with the VA and the community.

Responsibilities
  • Coordinate outreach efforts for the SSVF Program and establish an outreach plan targeting participants as outlined in program guidelines.
  • Screen potential program participants for program eligibility using VA‑approved forms and verify information with veteran and income status agencies.
  • Conduct housing and comprehensive supportive services assessments and develop a comprehensive Family Services Plan.
  • Provide service coordination between the program and community partners, including persons served and their families/supporters.
  • Work with persons served, family members, collaborating housing agencies, and professionals to revise service plans and develop placement, stabilization, and follow‑up plans.
  • Provide or arrange for housing support and wrap‑around services, including connections to eligible VA and mainstream resources for transition to or maintenance of permanent housing.
  • Travel to housing sites to support persons served, monitor progress, and address health, behavioral, or life‑skills issues.
  • Communicate effectively (under confidentiality rules) with clients, service teams, and staff to address issues rapidly and correct services or supports.
  • Ensure appropriate use of and documentation of any temporary financial assistance.
  • Ensure veterans and their families receive services that support rapid re‑housing and promote financial independence and relationships in the community.
  • Ensure participant category reassessments are completed according to SSVF requirements.
  • Participate in program and professional supervision, attend required staff and Project Management Team meetings and trainings, and contribute to team‑based collaborative planning.
  • Complete all appropriate reporting in HMIS and other systems as required.
  • Complete other duties as needed and requested.
Qualifications
  • Bachelor’s degree in counseling psychology, rehabilitative counseling, or related human services.
  • Experience working with homeless individuals and families, including veterans, is a plus.
Other
  • This position requires driving a personal vehicle for company business. Must meet Agency MVR guidelines.
    • Valid driver’s license in jurisdiction of residence.
    • Vehicle liability coverage if driving personal vehicle (may be required to transport clients).
    • May be required to drive a van.
  • Acceptable criminal background check, OIG exclusion check.
  • Negative drug screening.
  • CPR/First Aid certification must be acquired and maintained once employed.
Physical Requirements
  • Ability to safely operate a motor vehicle to transport yourself, consumers, and program supplies as necessary.
  • Physical ability to travel to assigned locations, stand, stoop, bend, reach, pull, push, lift, grasp, climb, talk, see, hear, and perform basic and light home maintenance activities, and operate office equipment.
  • Operating office equipment requiring continuous or repetitive hand/arm movements.
  • Ability to remain in a sitting position for extended periods of time.
