This position is provisionally (Grant) funded, benefits are eligible and subjected to grantor’s funding. Provisional through November 30, 2026, with the expected continuation of funding from the NC Division of Adult Corrections for the Local Reentry Council. The Local Reentry Council has been funded annually since its inception in 2016.

GENERAL DESCRIPTIONThe Case Manager will work directly with the Local Reentry Council Coordinator and program participants. The Local Reentry Council is an organized network of individuals and agencies that provide supervision and coordinate innovative responses to the reintegration of formerly incarcerated and/or justice involved individuals at the local level and through the use of new and existing program services. This position will work directly with individuals who are or have been justice involved to remove barriers that limit their success by providing case management, referrals, employment and housing assistance.
This position will serve as the primary point of contact for activities related to employment assistance and will be responsible for teaching job readiness classes.
DUTIES AND RESPONSIBILITIES- Complete intakes for justice involved individuals including orienting them to the program, obtaining signatures, establishing an initial case plan and entering their information into multiple information systems.
- Assist clients in developing and reaching goals identified on their case plan.
- Assist clients with assessing work history, strengths, employable skills, justice involvement, and other factors that relate to their current status and goals.
- Work with the Local Reentry Council Coordinator to grow resources;
Share housing leads, employment leads, and new resources to benefit all clients. - Provide assistance with housing and job searches.
- Provide intensive case management to clients seeking or maintaining housing.
- Transport clients as needed to community partners and/or housing.
- Identify training opportunities for participants to gain skills and knowledge for employment.
- Teach job readiness classes on site, assist clients with job searches.
- Participate in local reentry council meetings.
- Assist with the facilitation of employment subcommittee meetings.
- Assist with the preparation of reports for the Criminal Justice Advisory Committee and City of Durham related to housing support.
- Enter client information and document all client contact in required databases and spreadsheets to ensure accurate documentation of assistance.
- Participates in general and program staff meetings, and planning meetings; meets regularly with supervisor to discuss status, client and related issues and to identify action and next steps.
- Completes reports and records and other documentation as necessary and required.
- Acts as chaperone for urinalysis; provides front desk coverage as needed.
- Schedules and conducts intake interview and substance abuse screening on clients referred to the program; develops initial service plan with client and significant others to address identified needs; provides orientation for new clients admitted to the program.
- Consults with program staff, outside agencies or contract agencies and with probation/parole staff as appropriate; discusses and verifies employment status; gathers information for admission process i.e. judgments, arrest records, releases etc.
- Conducts Cognitive Behavioral Intervention (CBI) classes as scheduled and assigned; follow-ups with non-compliant clients and the appropriate referring agent.
