Position Summary
This individual will provide behavioral health support to residents of long‑term care facilities under the supervision of a licensed professional. The role includes receiving training in a broad range of assessments and interventions that support individualized treatment plans and requires regional travel.
Essential Duties and Responsibilities- Conduct assessments that include the behavioral and social needs of patients.
- Meet with families and facility staff to coordinate care as recommended by licensed professionals.
- Assess the patient’s knowledge of their clinical condition and provide education and self‑management support.
- Provide outreach to clients who are having problems удовольствием with treatment adherence.
- Assist in the development of the individual goals and behavioral health treatment plan.
- Coordinate with other agencies and community providers.
- Maintain timely documentation of work.
- Serve as an active member of the behavioral-health treatment team.
- Bachelor’s degree in Counseling, Social Work, Psychology, or a related field.
- Experience in a psychiatric or long‑term care setting preferred.
- Customer service experience preferred.
- Ability and willingness to function independently and proactively in a long‑term care setting.
- Effective use of electronic medical records and Microsoft software (Teams, Excel spreadsheets, etc.).
- Skilled in written and verbal communication.
- Skilled in exercising a high degree of initiative, judgment, and discretion.
- Ability to handle confidential and sensitive information.
- Communicate with others regularly, including exchanging information in stressful or high‑pressure environments.
- Move about the office to access files, use office equipment, and interact with others; sit or remain stationary for extended periods while working, communicating on the phone, utilizing the computer, and interacting with others; occasionally stand or otherwise access files, equipment, or items in high places.
- Operate computers, phones, and other office equipment continuously.
- Move by bending, stooping, kneeling, crouching, or crawling as needed.
- Noise level is usually low to moderate.
- Interact effectively with others in an office environment, manage conflict, and handle stressful situations and deadlines.
- Requires desk work in an office environment.
- Ability to work flexible hours as needed to meet deadlines.
- Maintain a respectful and professional demeanor at all times.
