Social Worker – Acute Hospital Discharge
Connect Exec are currently recruiting for six experienced Social Workers to join a City Council's Adult Social Care service, supporting the Acute Hospital Discharge Team.
This is an excellent opportunity for experienced Adult Social Workers who have a background in hospital discharge, community social care, assessments and care planning and are looking to take on a rewarding locum position.
The successful candidates will work as part of a team supporting adults to achieve safe and timely discharge from hospital, completing assessments and putting appropriate care and support arrangements in place.
The Role
As a Social Worker within the Acute Hospital Discharge Team, you will be responsible for completing strengths-based, proportionate and person-centred assessments and care planning for adults with care and support needs.
