Social Services Director is a high level Social Services Professional who will lead our interdisciplinary team. Working with therapy, nursing and the rest of the team to ensure the quality of resident care and to provide a smooth transition back into home life.

• Develop an ongoing care plan based on the residents’ needs, interests and abilities as identified in the initial assessment.
• Participate in care plan meetings.
• Complete required record keeping upon resident admission, discharge and transfer; chart and document per facility policy.
• Implement healthcare directives and identify end‑of‑life care opportunities.
• Assess the resident’s potential for discharge as a member of an interdisciplinary team.
• Plan for the resident’s discharge and post‑discharge with the resident, family and interdisciplinary team.
• Assist the resident and family in the implementation of the discharge plan.

• Bachelor’s degree in Social Work.
• 2–3 years’ experience in a social services/discharge planning setting in long‑term care or skilled nursing.
• Working knowledge of Medicare, Medicaid and insurance.
