Memory Care Nurse

We are seeking a compassionate and skilled Memory Care Nurse to join our dedicated team at Dyer Nursing and Rehab in Dyer, Indiana. As a Memory Care Nurse
, you will be responsible for providing high-quality care to our residents with dementia and Alzheimer's disease. If you are passionate about making a difference in the lives of others and are committed to delivering exceptional patient care,

• Provide personalized care to residents with dementia and Alzheimer's disease, including administering medications, managing behaviors, and addressing physical and emotional needs
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Conduct ongoing assessments and evaluations to ensure resident well-being and quality of life
• Communicate effectively with residents, families, and healthcare professionals to ensure seamless care transitions
• Participate in training and education programs to stay current on best practices in memory care
• Collaborate with colleagues to maintain a safe and supportive environment for residents and staff

• Current RN/LPN license in the State of Indiana
• Minimum of 1 year of experience in memory care
• Ability to work varying shifts, including days, evenings, and weekends
• Strong communication and interpersonal skills
• Compassionate and empathetic approach to care
• Ability to lift, move, and transfer residents as needed
