Job Title: Rounding Physician – Senior Living Primary Care (MD/DO)

Compensation: $225,000 – $250,000 annually

Location: Greater Twin Cities Metro Area

A dedicated Primary Care Physician (MD or DO) to deliver on‑site, relationship‑driven care to aging adults in senior living communities.

Established in 2016, Twin Cities Physicians now serves over 4,000 patients across 200+ senior living communities in Minnesota. Our care model brings compassionate, coordinated primary care directly to where our patients live, reducing unnecessary ER visits and improving outcomes.

• Conduct regular rounding visits in assisted living and memory care environments
• Perform comprehensive medical assessments and manage complex conditions
• Develop personalized care plans in collaboration with patients and families
• Work closely with NPs, PAs, and care teams to ensure seamless coordination
• Educate residents and their loved ones about care options and goals
• Proactively manage chronic conditions to reduce hospital admissions

• Active, unrestricted MD or DO license in Minnesota
• Board certification in Internal Medicine, Family Medicine, or Geriatrics preferred
• Prior experience in senior living, rounding, or Hospitalist medicine
• Strong communication and decision‑making skills
• Valid driver’s license and auto insurance
