Under the direct supervision of the Practice Development Manager, the Patient Coordinator will manage the clinical support coordination of patient care. This includes scheduling follow-up appointments, surgery coordination, communication across care teams and other departments, and supporting patient care activities as directed by physicians and providers.

This is a full-time Patient Coordinator position providing primary support to a team within the Arthritis Service Line, primarily at our Eagan and Lakeville campuses, with flexibility to float to other locations as needed. The schedule is Monday - Friday, 7:30 AM to 4:00 PM, subject to change based on clinic needs. Coverage at Summit outreach sites may also be required.

Typical

duties include:
  • Answering and responding to patient phone calls, managing team voicemail and email, and triaging calls appropriately.
  • Managing physicians' daily schedules, including clinic and surgery schedules, and supporting urgent patient needs.
  • Supporting coordination of patient care between team members and other departments.
  • Communicating information from physicians and care providers to patients and staff.
  • Maintaining worklists and accurately entering physician orders into the electronic medical record.
  • Completing standard patient forms (e.g., FMLA, Short Term Disability, Workers Compensation) as directed by physicians.
  • Acting as backup to other teams, including handling voicemail, email, and tasks.
  • Assisting with rooming patients and initiating visits as directed.
  • Performing other duties as assigned.
  • Our hiring range for this position is $21.19 to $26.49 per hour. The final salary will be based on skills, education, experience, and internal equity. Benefits include medical, dental, vision, disability, life insurance, paid time off, and a 401(k)/profit sharing plan. Additional compensation such as bonuses and differentials may apply.
