Thames Reach runs projects and services that help to end homelessness in and around London. We work directly with homeless people on the streets and in hostels. We work to prevent vulnerable people from becoming homeless. We help formerly homeless people get their lives back on track. We help people find work and access training and volunteering opportunities.

About this role…
We are looking for a Lead Worker to join the Ealing Move on Team.
Thames Reach’s Supported Housing is comprised of Accommodation, Housing First, Navigator and Move on services supporting people to make real and lasting change in their lives. People may present with complex mental health, offending histories, drug and alcohol needs.
Navigator and Move On services:
Provide holistic floating support to clients at different stages of their homelessness journey helping them navigate and access systems and services with the ultimate aim of securing accommodation in the community and prevent future homelessness.
We will not accept applications where Artificial Intelligence (AI) has been used to assist with completing the competency questions.
You will:
- Conduct comprehensive assessments to identify individuals’ support needs and develop personalised, outcome‑focused Move‑On Plans.
- Work autonomously within the community, using professional judgement to deliver effective, person‑centred support.
- Represent the service at internal and external meetings.
- Use creative and proactive problem‑solving to address challenges, reduce risks, and enable positive progress.
- Promote a whole‑team approach, working closely with colleagues and partner agencies to help people achieve their goals and build long‑term aspirations.
- Maximise income and improve wellbeing by supporting individuals to access welfare benefits, health services, and wider wellbeing initiatives.
You will have:
- Experience working with vulnerable individuals with diverse support needs, using a person‑centred and outcome‑focused approach to help them sustain their accommodation.
- Confidence in lone working within a floating support service, demonstrating reliability, good judgement, and strong organisational skills.
- Ability to maintain accurate records, including effective data collection and the upkeep of an up‑to‑date case management system.
- Strong collaborative skills, with the ability to work effectively with service users, colleagues, and a range of internal and external partners.
- Excellent communication and active listening skills, with the ability to build trust and engage people meaningfully.
- A compassionate, empathetic, and trauma‑informed approach, ensuring support is delivered sensitively and respectfully.
