This is an exciting opportunity for a skilled Social Worker to join the Hospital Discharge Team in Ealing. You will be part of a dynamic team working within a variety of healthcare specialisms while earning an attractive rate of £34 per hour. You will support the St George’s Hospital Stroke Unit, providing vital discharge planning and social care expertise.

What you will do:
  • Manage a caseload of patients, collaborating with a multidisciplinary team for comprehensive care.
  • Provide guidance on social care processes, ensuring patients and families understand available support options.
  • Proactively engage in discharge planning, anticipating obstacles and addressing them early in the process.
  • Connect patients and carers with support services related to benefits, housing, and more.
  • Coordinate with external agencies to solve issues such as homelessness or lack of public funds.
  • Contribute to the efficient management of patient flow across inpatient pathways, maintaining high quality care.
  • Mentor the team on navigating social care roles and the implications of The Care Act.
  • Support complex stroke case discharges with the Neuronavigator and other teams.
  • Deliver clinical expertise to external agencies as required.
