This is an exciting opportunity for a skilled Social Worker to join the Hospital Discharge Team in Ealing. You will support the St George’s Hospital Stroke Unit, providing vital discharge planning and social care expertise.

• Mentor the team on navigating social care roles and the implications of The Care Act.
• Support complex stroke case discharges with the Neuronavigator and other teams.
• Deliver clinical expertise to external agencies as required.
