Home Health Aide at 219 Health Network

The Home Health Aide is a trained individual who provides personal care and related services to patients in their home to promote dignity and independence as prescribed by the attending physician. The Home Health Aide provides services as directed by the Home Health Case Manager and is responsible to the Clinical and/or Care Manager.

• High School diploma or GED required.
• Successful completion of Certified Nurse's Aide/Home Health Aide training totaling at least 75 hours, including at least 16 hours of supervised practical training.
• Valid registration/licensure with Indiana State Home Health Aide Registry Program.
• Successful completion of written and skills competency tests prior to patient care.
• Previous clinical nurse aide experience preferred (one year in hospital, long‑term or rehab setting).
• Agrees to have criminal investigation with results complying with IC‑16‑27‑2 Standard.
• Valid driver’s license and proof of current automobile liability insurance.
